Play Area Injury Waiver

Participants agreement including assumption of risks and agreements of release and indemnity. Read this Agreement carefully. It must be signed (clicking yes to all statments, typing full name of parent/legal guardian and clicking "submit" form) by the Parent/Legal Guardian of the minor. Waivers are good for one year as indicated on the date this form is filled out.

For the purpose of this form, the child listed on this form is hereinafter Minor and Cowboy Fellowship of  Atascosa County, Inc. is hereinafter Cowboy Fellowship.

All minors inside the play area must have a waiver on file whether they are participating or are spectators. Any minor not on the Cowboy Fellowship official roster may not enter until a waiver is signed or re-newed by parent/legal guardian. The Authorized Representative of the Renting Organzation under a renters contract for use of the play area is responsible to ensure all waivers are signed prior to entering the play area.

If you do not wish the minor to participate inside the play area and the minor attends youth/kids church functions, please contact Cowboy Fellowship Children's Ministry Staff. Do Not Complete This Form. Renting organizations will not permit minors inside play area as a spectator unless a waiver is on file.
