Play Area Injury Waiver

Participants agreement including assumption of risks and agreements of release and indemnity. Read this Agreement carefully. It must be signed (clicking yes to all statments, typing full name of parent/legal guardian and clicking "submit" form) by the Parent/Legal Guardian of the minor. Waivers are good for one year as indicated on the date this form is filled out.

For the purpose of this form, the child listed on this form is hereinafter Minor and Cowboy Fellowship of Atascosa County, Inc. is hereinafter Cowboy Fellowship.

All minors inside the play area must have a waiver on file whether they are participating or are spectators. Any minor not on the Cowboy Fellowship official roster may not enter until a waiver is signed or re-newed by parent/legal guardian. The Authorized Representative of the Renting Organzation under a renters contract for use of the play area is responsible to ensure all waivers are signed prior to entering the play area.

If you do not wish the minor to participate inside the play area and the minor attends youth/kids church functions, please contact Cowboy Fellowship Children's Ministry Staff. Do Not Complete This Form. Renting organizations will not permit minors inside play area as a spectator unless a waiver is on file.

I understand that the activities inside the play area include bouldering (a type of rock climbing that does not utilize ropes or harness to climb a wall with a height of less than 16 feet) and a multi-level padded playground (includes slide, balance obstacles).

I understand and have read the rules posted for use of the play area to include the Playscape and Climbing Wall.

I understand that the activities require moderate to heavy physical exertion, and I represent that the minor does not have any mental or physical condition that might create risks to ourselves or others.

I understand that climbing and other activities in the play area are dangerous, and my child will be exposed to risks including slips, trips, falls, abrasions or collisions. Falls from certain positions can result in serious injury regardless of floor padding. Parents must carefully access the childs competencies, in climbing and falling, practice down-climbing as a preferred alternate to jumping, and increased climbing heights and complexity of moves thoughtfully.

I understand that the risks described above, and others, inherent or not, may result in all manner of trauma including breaks, sprains, abrasions, serious injury and even death. I acknowledge and voluntarily assume all such risks for my child, inherent or otherwise, known or unknown, whether or not described above.

I, the undersigned parent or legal guardian, agree not to sue, and to release, discharge, defend and indemnify, Cowboy Fellowship.

I certify that the information given on this form is correct and have read and understand this agreement. I also understand that it is my responsibility to inform Cowboy Fellowship of any changes that should occur during the duration this form will be utilized .
