General Information
1. Do you have any physical limitations, dietary restrictions, or medications that would need to have a doctor advise you on before the trip?

2. Will you be able to attend all training requirements leading up to the trip, as well as post trip debriefing?

3. Do you have any safety concerns about travel or the destination country you want to travel to? If so, what are they?
4. The trip will be physically, mentally, and emotionally demanding. Long flights, rough car rides, remote locations, different foods, and plenty of walking over different terrain. There will be limited access to medical facilities, internet, and little comforts from home for the duration of the trip. Are there any physical, mental, emotional, social, or spiritual issues or concerns that could arise during the trip that would keep you from being a part of the team and mission?
